Instructions to Receive Payment
Below is the form and information required to be submitted by JUNE 6, 2024 by recipients for the 2024-2025 academic year before payments will be made to your school.
PLEASE NOTE: This process has changed as of the 2019-2020 academic year, if you were a recipient in years prior to 2019 please read the directions thoroughly. If you have questions about a previous scholarship award please contact Abby King. Students who received a scholarship in the 2023-2024 academic year should submit their final Fall 2023 and Spring 2024 grades together to Abby King. It does not need to be an official transcript; it can be a screenshot from your online portal.
PLEASE READ CAREFULLY
Please fill out the information on the form and return before June 6, 2024 by mail, email, fax or upload via link if awarded online. No payments will be made until this form is received. If this form is not received by the deadline the scholarship may be forfeited in its entirety.
Payment will be directly mailed to your school on your behalf for credit to your student account to be applied to tuition charges after Aug 3, 2024. We can only make payment if your school is accredited. The school is instructed to apply the scholarship funds evenly over the 2024-2025 academic year. Please contact our office if you are not going to be a full-time student for both Fall and Spring semesters of the 2024-2025 school year for further instruction. We are not responsible for late fees. The Fairfield County Foundation is not permitted to reimburse families or individuals for prepayment of fees. This is a tuition-only scholarship and you must be a full-time student. Scholarships are awarded for the 2024-2025 school year only and are not renewable. You may reapply for this or other scholarships from the Foundation in upcoming years. Please check our website for future application dates and deadlines.
You must send the Fairfield County Foundation your grades for Fall 2024 and Spring 2025 together after completing the 2024-2025 school year in order to maintain your eligibility.
Please be sure to review the criteria for the scholarship you have been awarded and notify us of any changes in your educational plan that may affect your eligibility to receive the scholarship.
If you were awarded a scholarship from our online system please log back in to complete the follow up form assigned and answer the supplemental questions. If you were awarded a scholarship in another manner, information should be submitted to Abby King via mail, email (aking@fairfieldcountyfoundation.org) or fax (740-654-3971).
